Recognize common cognitive biases to make the most out of incident investigations

Most companies conduct incident investigations to try to glean lessons learned and improve safety for the future. Yet, actual improvement resulting from incident investigation reports often falls short of the total potential for improvement due to cognitive biases. In this video, Julia Burggraaf, Product Manager for CGE Risk Management Solutions, explains the most commonly identified biases – outcome bias, hindsight bias and curse of knowledge – and how they influence incident investigations.
